Description:
The Case Manager 1 provides case management services to Enhanced Care and Enhanced Care Outreach clients. This position assists clients in accessing services, and collaborates with add Community Partners and Agency professionals in case planning and service coordination.
Mid-Columbia Center for Living (MCCFL) is a Certified Community Behavioral Health Clinic (CCBHC) that specializes in providing services that are both trauma-informed and recovery-oriented to persons / consumers with mental illness and addictions. Trauma-informed care includes safety, choice, collaboration, trustworthiness and empowerment for both clients and staff. MCCFL serves Hood River, Wasco, and Sherman Counties and utilizes a consumer-involved, consumer-engaged, and consumer-driven model. MCCFL provides a whole-person care approach to adults and children to address both physical and mental health needs. MCCFL offers a variety of programs including a mobile crisis unit, intensive case management, ongoing counseling, community support, supported employment, jail diversion, hospital discharges, hearings, and more.

What skills and education you need:
Incumbents in this classification must meet requirements as a Qualified Mental Health Associate (QMHA) and certified as such by the Mental Health and Addiction Certification Board of Oregon (MHACBO). Any combination of experience and training equivalent to the following is qualifying:
- Bachelor’s degree in social work, psychology or related human services field AND two (2) years’ responsible experience in human services; OR
- High school diploma and five (5) years of equivalent training and work experience in human services.
- Experience working with the severe and persistent mentally ill and geriatric populations is preferred.
- Must possess a valid Oregon or Washington Driver’s license and be insurable for client service purposes and for travel between business offices, meetings and/or trainings.
- Must pass all criminal history check requirements as required by ORS 181.536-181.537 and in accordance with OAR 407-007-0200 to 407-007-0370.
What you’ll do:
- Provide case management services to all assigned clients. Services are provided in a variety of community settings, with consideration to client preference.
- Assist clients in acquiring services and resources needed to achieve treatment goals and maintain independence within the community. Coordinate with other community partners to develop, provide, and maintain a rich array of services and resources for clients. Provide transportation for clients when required.
- Actively participate in joint case planning and coordination with internal multi-disciplinary team and agency psychiatrists.
- Meet minimum client service productivity requirements as may be set by the Agency to meet requirements of outside funding and/or reimbursement sources.
- Utilize agency clinical software system to prepare current client records and reports as necessary and required. Maintain timely and cogent progress notes and plans. Ensure that client records and other documentation are up-to-date and justify the case management services provided.
- Adhere to all clinical and MCCFL program policies and procedures. Remain updated on all applicable rules and OARs.
- Participate in all team and agency meetings as scheduled. Attend local, regional, and/or state meetings as requested by Supervisor/Program Manager. Within the scope of the position, perform duties independently in the community and promote a positive image of the agency.
- This position may require travel between Hood River and multiple locations in the community.
- This position requires 8:30 to 5pm schedule with work days of Saturdays and Sundays and 3 weekdays to fulfill the 37.5 hours per week.
- Regular and reliable attendance; professional communication with all contacts; confidentiality
- Program functions 7 days a week, weekend work required.
- Primary work location is Hood River Care Center and sometimes in the Community Room at MCCFL offices in Hood River.
- Focus populations are SPMI and geriatrics.
- Daily psych therapeutic groups.
- Close work/collaboration with Hood River Care Center staff (nurses, CNAs, administration, all departments).
- Development of specific behavior plans using positive reinforcements.
